To make an appointment please call the office location of your choice. If this is your first visit, you will need to fill out the New Patient paperwork.


You may fill the paperwork out by printing the New Patient Information Form and the Patient Questionnaire form. Please complete both forms and bring them with you to your visit. Or if you prefer, come early for your appointment to allow time to complete this paperwork.


For your and our protection, we will not treat a minor unless accompanied by a parent or guardian. A minor is anyone 17 years of age, or younger, with the exception of a married female. 

Financial Policy

We thank you for choosing Arkansas Otolaryngology Center. We ask you to read our standard Financial Policy prior to your visit. If you have any questions, please contact our office prior to your visit. 

Please bring your current Insurance Card and Driver’s License with you at the time of your appointment. We participate with most insurance network plans, including Medicare, Arkansas Medicaid, ArKids, Tricare, Aetna, Cigna, Blue Cross and Blue Shield, Health Advantage, NovaSys, QualChoice-QCA, United Health Care, etc. If your insurance plan is not listed, please contact your insurance company to verify our doctor is in your particular network. 

Insurance plans (HMO, POS, etc.) that do require a referral from your primary care provider (PCP) are very strict in that requirement. If your insurance plan requires a referral from your PCP, please bring that referral with you. Any services received without the insurance-required referral will be your responsibility. 

We will be happy to file your insurance; however, co-pays and co-insurance amounts are to be paid on the day of your visit. After we hear from your insurance, you will receive a statement from us indicating what your insurance has paid. Any remaining balance is due upon receipt of that statement. 

No Insurance / Self-Pay
Payment will be due at the time of service. If you are unable to pay your balance in full, you will need to make prior arrangements with our Patient Accounts Department. 

Credit Cards Accepted
Visa, MasterCard, Discover, American Express, Debit Cards and Care Credit are welcome. 

Patient Privacy
We respect our patient’s privacy and comply with all federally-mandated privacy (HIPAA) policies. You may read our Privacy Statement located in our Front Desk tab. 

Release of Medical Information
For medical records to be sent to AOC:
To request that your records with another healthcare provider be sent to us, please go to the Front Desk tab, choose Medical Release from AOC, complete the form and deliver or mail the request to the provider from whom you are requesting records. 

For medical records to be sent from AOC elsewhere:
To request that we send your Arkansas Otolaryngology records to you (or a 3rd party), please go to the Front Desk tab, choose Medical Release from AOC, complete the form and deliver or mail the original signed form to us. There may be fees associated with copying and sending your records, as allowed by Arkansas state statutes. Because we use an outside records copying company, please allow up to 10 days for processing. 

Prescription Refills
Please call your pharmacist for refills. Your pharmacist will call our office to get your physician’s approval. 



(Established Patients Only)
Please call our offices and you will be routed to a Nurse Message Line to leave a message. If you call before 3:00 PM, every attempt will be made to return your call before the clinic closes that day. Otherwise, your call will be returned on the following business day. If you do not hear back from us within this timeframe, please call us back.
